Form W-9

What is Form W-9?

Form W-9, “Request for Taxpayer Identification Number and Certification,” is a United States tax form used by individuals and entities to provide their taxpayer identification number (TIN) to entities that will pay them income during the tax year. The primary use of the form is to help payers or payees obtain accurate information required for various types of income tax reporting to the IRS.
